Parents of students enrolling at Concordia Lutheran School must present tangible documentation that immunization and health requirements have been met for your child/children. This documentation includes the HRS FORM 340 and HRS FORM 680. These forms are standard in all pediatrician offices and must be received by the school office staff within two weeks after the start of school.
