Change of Address and
Contact Information
The office should be notified immediately regarding
any change of home address, telephone number, and/or
the parents’ place of employment and phone number.
All pertinent information such as a change in doctor,
dentist or a student’s allergy to a specific
medication should be made available to the school
office. Up-to-date information may make a difference
in the welfare and care of your child/ children in
an emergency situation.
